PAC Mate Omni Upgrade Form

This form can be used to submit a request to Freedom Scientific for a PAC Mate Omni™ upgrade. After you have submitted this form, Freedom Scientific will contact you with an RMA (Returned Authorization Material) number and the date reserved for your PAC Mate upgrade. If we do not receive your PAC Mate by the scheduled due date, you may lose your place in the queue and encounter undesirable delays.

Only units received in good working order will be accepted for upgrade. Units that are not received in good working order will lose their place in the upgrade schedule. Other arrangements will need to be made to repair the unit. It will not be rescheduled for upgrade until all necessary repairs are completed.

All fields listed with an asterisk (*) in front are required. Please submit one form for each individual unit.

PAC Mate Omni Upgrade Process

An RMA number will be provided to you within one business day once the request is received and processed, and all information is provided. The preferred method for providing you with this number is via e-mail, which results in faster service. Please make sure to enter an appropriate and accurate e-mail address in the e-mail address field below.

English language upgrades will be available first, with other languages to follow.
